Will a randomized trial of unruptured brain arteriovenous malformations change our clinical practice?

چکیده
try to force another (“last”) coil in, with subsequent risk of the need for retrieval. With these technical precautions, unraveling of a coil during withdrawal will be rare. We believe that the drawback of possible coil stretching and unraveling in standard coils without stretch resistance is only a minor clinical issue that is outweighed by the shortcomings of the SR filament in terms of handling, safety, and obtained packing attenuation. Standard coils are hardly available on the market any more. We plead for a renewed appreciation of the better physical properties of standard coils without SR filaments, so that operators can choose between standard or SR coils in every coil type.

Risk Reduction of Cerebral Stroke After Stereotactic Radiosurgery for Small Unruptured Brain Arteriovenous Malformations.
BACKGROUND AND PURPOSE A Randomized Trial of Unruptured Brain Arteriovenous Malformations (ARUBA) indicated the superiority of medical management in reducing the risks for strokes and other neurological deficits over observation alone. The aim of our study was to verify the rationale for stereotactic radiosurgery (SRS) for small unruptured arteriovenous malformation. METHODS A retrospective r...
متن کاملA randomized trial of unruptured brain arteriovenous malformations (ARUBA).
Despite almost a century of efforts in the treatment of brain arteriovenous malformations (BAVMs) no clinical trial has yet been performed to demonstrate the benefits of intervention versus conservative (medical) management for those not yet bled. Only insufficient information on the natural history of unbled BAVMs exists to certify that intervention is needed and that such intervention produce...
متن کاملUnruptured brain arteriovenous malformations: another asymptomatic conundrum.
There are 3 major asymptomatic cerebrovascular diseases that challenge the stroke clinician at the bedside. These are asymptomatic carotid disease, unruptured cerebral aneurysms and unruptured brain arteriovenous malformations (AVMs). In all 3 conditions, there are uncertainties about natural history, risks and benefits of intervention. While controversy remains, there have been 2 trials that h...
متن کاملRadiosurgery for Cerebral Arteriovenous Malformations in A Randomized Trial of Unruptured Brain Arteriovenous Malformations (ARUBA)-Eligible Patients
Cerebral arteriovenous malformations (AVMs) are rare vascular lesions detected at an annual incidence of ≈1 in 100 000. The natural history of unruptured AVMs is more benign than ruptured ones, primarily because of their lower hemorrhage risk. The only currently available strategy for AVM management that provides durable relief from the risk of hemorrhage is complete obliteration of the nidus. ...
متن کاملRecent Trends in Neuro-endovascular Treatment for Acute Ischemic Stroke, Cerebral Aneurysms, Carotid Stenosis, and Brain Arteriovenous Malformations
The efficacy of mechanical thrombectomy with stent retrievers for emergent large vessel occlusion has been proved by randomized trials. Mechanical thrombectomy is increasingly being adopted in Japan since stent retrievers were first approved in 2014. An urgent clinical task is to offer structured systems of care to provide this treatment in a timely fashion to all patients with emergent large v...
متن کاملRadiosurgery for Cerebral Arteriovenous Malformations in A Randomized Trial of Unruptured Brain Arteriovenous Malformations (ARUBA)-Eligible Patients: A Multicenter Study.
BACKGROUND AND PURPOSE The benefit of intervention for patients with unruptured cerebral arteriovenous malformations (AVMs) was challenged by results demonstrating superior clinical outcomes with conservative management from A Randomized Trial of Unruptured Brain AVMs (ARUBA). The aim of this multicenter, retrospective cohort study is to analyze the outcomes of stereotactic radiosurgery for ARU...
